How does this secure the tire if the bolts to the lock bracket are on the outside of the rim of the tire ?

When you apply a padlock (etc.) it prevents turning those two bolts, so it is reasonably secure.It won't prevent someone from breaking/cutting off the lock, or unbolting the entire assembly from your bumper. But then, for this price you get a good deterrent, not a 100% foolproof prevention device.

What is the distance from baseplate to tire mounting plate. Using this to mount a spare on an element and need this measurement for tire clearance.

5 5/8" Base plate to tire plate. On my boat trailer with A78 13 tire there is 2 7/8" by turning the tire over with the inside of the wheel facing out you can get an extra 1 7/8"

how is the spare tire secured? it seems as if i remove both lug nuts the security doesn't seem to help!

You put the tire on first. Then you screw on the brackets for the padlock. Line them up and put the paddle in, close and locked. Tire is secure.

when measuring the holes for distance apart, its center to center ? correct?

One answer says to measure from the outside edge of one lug to the center of the one across. Question - on 5 bolt pattern, do you consider either "across" bolt to measure from? As neither bolt is directly across.
