Q: how do you attach the ratchet heads?- I can put in the screwdrivers heads but not the sockets

Look in the row containing the screwdriver heads - there's one piece which is an adapter, it has a small ballbearing in it. You can slot this into the handle then push a socket onto the adapter and the ballbearing keeps it in place.

Q: Do you have to use the extension bar or can you fit the bits into the driver direct?

The "extension bar" is part of the handle

Q: Does this set have a 2mm hex key in it?

Sorry no it doesn't. Has Phillips, flat and star keys.
