Q: Is the foil backing aluminum or is it some kind of paper/aluminum stuff?

It is metallized plastic, NOT aluminum foil. Also it is flammable (burns when exposed to a flame).

Q: Is it rated for 220 degrees?

The product doesn’t specify any temperature range. I’d be careful of anything that hot. Product is designed for water pipes.

Q: Can this be used outdoor pipe exposed to multiple variety of weather conditions?

Probably not. The foil backing is only on one side and is pretty flimsy, and the fiberglass insulation will get soggy in the rain.

Q: Is this appropriate insulate a ceiling register box?

I don’t see why not. But this is for pipes and is only 3” wide so you’d use quite a bit to wrap a register box depending of course on how big the box is.
