No. You might feel a tingling or a warming sensation but I felt nothing outside of my foot pain improved over time.

The smooth side touches the skin. But I don't think it matters all that much, since you can put these under your sheets for the same effect. I have one under my keyboard. I cannot report that I have noticed a change. I've been using it for several months.
