I wish to use this with an amcrest ip4m-1053e camera which is poe+ but also has 12v dc plug. what splitter is required (or possible with no splitter)?

The whole point of POE is to supply data and power in a single cable. With a POE camera, you won't need your 12v plug - the Cudy POE injector will provide the power for the camera through the ethernet cable.

Can this device connect to a poe nvr on its input? basically, i need more power than my nvr can provide.

You can not use this to add input power to a nvr however, you can use it to power the end of line devise.

I am using this with a reolink rlc410 poe camera. looking at the spwcs it should work fine, but when i plug in the the ethernet cable it doesnt work.

I'm using this injector to power a Reolink RLC-423, over roughly 10m / 30' of shielded cat6 cable, and it works great. How long is your cable run, and are you using cat5e or cat6 (or higher) ?
