Q: how do you discreetly retrieve your items while wearing so no one else sees what you are doing?

You don't. When you retrieve money, pull it from some other place. What you might place here is the credit cards or ID you don't plan on using that day. If your other source gets robbed, then you have this as a backup. Unfortunately, if you are small busted like me, this shows through. You have to wear a sweater or something heavy over it.

Q: Could this attach to the shoulder strap of a bra for easier retrieval?

Yes the pictures I saw and the look of the actual item you can attach it anywhere on the bra. It is a fabric loop. I attach mine to the strap and tuck the pouch into my bra cup. I would not be comfortable leaving it dangling under my clothes. I am paranoid that way. Hope this helps.

It lays very flat, unless you stuff it full. I just had a hotel room key in mine.
