Q: Can you wear this without shoes?

I did not use a shoe, just a pair of socks.

Q: I can't tell, does the soft pad come with both left and right afos?

Don’t understand the question, but you must specify the right or left side when ordering. You will need to use a larger shoe side to use this brace. My husband used the adjustable drop foot brace. Made out of velco, very comfortable and can be use on either foot. I hope this was helpful
