I have a 5/8 male end hose do I order the 3/4 female adapter?

All the outdoor water hoses that I have used have the same ends whether they are 5/8 or 3/4 hoses. I had 5/8" hoses and these fit fine.

Both sides seem to be the same thing, what is the purpose of this adapter?

If they were not the same you may not need an adapter at all. This is a female to female hose thread adapter. There may be instances where you need to connect two male hose ends together. The most common instance is in a hose storage reel. Hose reels commonly have a male adapter at their input valve. In order to connect a reel like this to a spigot a special female to female hose (sometimes called a feeder hose) is required. If you don’t have or want to by a single purpose feeder hose, this adapter will allow any standard (male to female) garden use to connect a water source to a reel.

Do these swivel or are they fixed?

They do not swivel, it allows you to screw each side on to the hose.
