Is the drain assembly and hose included?

Yes, but I don't plan to use either the faucet or the drain assembly and hose that were provided. The look is great, but the stainless is thin, and I wish they'd put the money on better steel and not bothered with the fittings at all. Other reviewers alerted me to these issues, but I went for it because the shape and size (only 34" tall, a liability for some!) and I think 19" deep. Perfect for my app. First one I got had been partially installed and returned, but they promptly sent a replacement, also flawed. Still, I was able to cobble together a perfect unit with parts from each, and sent back the damaged unit. Good operation, I think, fwiw.

Did anyone replace the drain hose and if so what did you purchase? Mine is rotted out so I need a new one and I don’t even see where to contact seller

Is the center shelf adjustable

No, and this sink has plastic parts including drain and hose (ptrap) it is not proper. I bought all parts aside including faucet, drain and waste line. Now I have to fill flimsy tube legs with something to make it solid.

How can this be secure to the concrete floor?

I secure it to the wall, similar to the way you attach the shelf to the wall. Just be creative about it, no instructions or supplies included in the package for it.
