mine were smooth and primed ready to paint.

The Screws are just for show, and yes, they do come with the item. What actually holds them on is the screws holding the fender wells to the rim of the wheel well. I thought it sounded flimsy at first, but once I put them on, they held pretty solid. All hardware is included.
