Pulling on the paper towel causes more to come out. You can tear it off and more will roll out or lightly pull and get a longer length before you tear it off. There's a bar just inside the towel outlet that seems to activate it. If there is no towel, reaching just inside at the bottom and tapping it will roll a towel out to get it started.
