There is only a clamp that attaches to it.

Q: Can I get a replacement nut so that I can secure the swivel?

You may have to go to a local hardware store and buy a metric nut. Take the bolt or the other nut with you if there’s a second nut on the fan.

Q: Is it sturdy to cool my dog on the back seat?

Yes, this fan for car is very sturdy material. It is very suitable for use in the back seat for pet.

Q: Can this be used for a cement truck ?

I don’t see why not. I clip mine on the rear view mirror mount on my Dodge Ram 2500. Work’s good and much better than the others.
