Yes to all. And the ones I purchased came with hard floor casters.

Hello Brian - Wendy from Office Designs here - This particular model on Amazon does come with hard floor casters, but not with breaking casters. Unfortunately, we do not sell this chair model with breaking hard floor casters on Amazon.

Q: Does it come Assembled?

The back was not attached but only required removing the two bolts from the frame, sliding the back in place and installing the bolts. It took all of 5 mins - if that much. You will need a hex tool to tighten the bolts. Once complete, there is a cover that goes over the bolt access area. Also, the casters are new and were in a box. You simply push them in the legs.
