I lay down and put it first on my neck and hold on to either side to give a little traction and weightlesssness to my head, twisting with my forearms. Once I've judged it to be in the right position, I slowly and carefully with plenty of room push down with my feet against the floor until it's square between my shoulders where it's most effective. If you were to push too hard or move too quickly I could see it being less than comfortable but if you're slow and careful it's awesome. I'll finish by rolling it maybe 3/4 of the way down my back..it's no good for me at the base of my spine..but otherwise it's my favorite de-stresser!

back bliss, but go easy!
I was looking for the eco-friendly solution to a foam roller for easing tension before and after exercise and went out on a limb with this one. For my wife, it's a bit too broad at around 1 1/2 feet wide but for me who's on the small side of a medium build, it fit perfectly between my shoulders where it feels absolutely perfect! Not what I expected... should have come with info on using it
I assumed The back massager.. ( my mistake by assuming ) that it would have some type of instructions that came with it to show you how to use it safely and effectively. However, it came in a plastic bag with nothing but the back massager... I saw more info on line from other companies, when I first was looking for something for my back that I could do without injuring myself more. What I saw on other sights I just assumed that Amazon would send the product with instructions. I actually was going to send it back because I have no idea on how to use it. But, we have had some unexpected illness in my family and I have been busy taking care of that, and didn't realize that a month had gone by. It is still in the plastic bag and never been used.I have never been disappointed in the past, but this purchase was a waste of my money since I have no idea how to use the product safely without causing further injury to my back.I would NOT recommend this product unless it came with instructions in the future.. Oh and if you decide to do that, please send me a copy of those instructions as well... They would come in handy...

Thankful for the Reviews that Helped Me Decide to Buy This
I'm really glad I bought this roller (Maxi Backsie). For such a simple tool, it is really versatile. I have a specific spot in my back that pops out of place when I am sitting for any length of time. I've always used a pillow for support (and tried many kinds), but this roller lets me support that exact spot that pops out. I often use it in place of a support pillow. Surprisingly, it isn't uncomfortable to use it this way and the roller works with most chairs.I also use this roller to gradually release my back as it is intended to be used. Sometimes this works great, other times if I am too stiff, it hurts too much. Overall, I love it.

Splits
I have used several Ma Rollers in the past. This product worked fine when I first purchased it in 2009 and it still does, but soon after purchasing, it began to split. The split grew more noticeable and now it is up to 3/16" wide at its widest point. However, I still am able to roll on it and it has not split in-two as of 2015, so I figure that I have gotten my money's worth even if it ever does.

Didn't match picture
The item I received did not match the picture shown on the site at the time I ordered (and the time of this review). The item pictured has rubber rings on the ends of the roller and smooth, angled slopes from edge to inner track. The item I received had no rubber rings and a ridged flat slope from edge to inner track. Consequently, the roller I received slipped easily and after several uses the ridged portion began to cut my back. Home Naturals Inc.'s customer service rep was polite, but not terribly helpful. Amazon's customer service rep was fantastic though and resolved things to my satisfaction. I subsequently ordered the Body Back Company Back Roller sold through Amazon directly (which has the exact same item pictured for a couple of bucks more - unsurprisingly Amazon actually sent the item pictured instead of a poor substitute ;-) which I've found to work very nicely.

Relief for areas you can't reach yourself.
Bought this on recommendation of a friend. At first it hurt a little using it, until I found my balance on it. Great for lower back, and I use it to loosen up my glutes.

Chronic Back Relief
I have a multitude of back issues, and the spine aligner has helped me work through them.I recently had surgery for a herniated disk, and this has allowed me to start to release the super tense muscles in my back.This is not a comfortable thing to use, it hurts, but in a way that you can tell is good (if that makes sense).I love this and will not travel with out it. I now keep it in my office and use it at work when my back gets stiff/knotty from sitting.
