How can I lock it from inside without the phone ? I set it to auto-lock after 6s. It says "Locked" but in fact it never locks.

You can’t. If it isn’t actually locking when it says it is, then there is a malfunction with the lock. It is an odd thing tho, the handle still moves when the door is locked. So, make sure that you are testing that it is locked from the outside, with the door shut or it will always seems as tho it is unlocked.

Every other day when I go to unlock the door it says operation failed but gives no explanation, my key code or key fob will not work also

I would return it. Mine works completely fine.
