Q: Does this come in a double outlet option?

It comes with one duplex plate (screw in middle) and one decora plate (rectangle, one screw top and one screw bottom).

Q: the outlet place appears to be slightly longer than the opening on the shelf. Is it supposed to be this way?

How can that be? Aren’t you just replacing an existing switch plate cover?

Q: Will a standard plug fit through the opening for the cord? my electric toothbrush doesn't have usb, just a regular wired to the charger.

Yes. There is a large enough hole through the assembly to feed the cord through and coil it around to hold it in place.
