Q: how long does it vibrate for?

As long as the pillow has pressure like a hug or leaning on it.

Q: If held, can this stay on for 2-3 hours?

We have never tried that - I can't say. It vibrates pretty fast - I think it would be a challenge to hold on to it for that long.

Q: How do you clean this pillow?

It has a waterproof cover with a zipper. So you remove the cover, wash it in the machine and put it back on the pillow. I used lysol on the inner pillow before putting the cover back on.

Q: Simply turning the switch to "on" activates my pillow, no pressure is needed. I have to turn the switch to "off" to stop vibration. Is this right?

On mine it has to have pressure against it to make it vibrate. I put it behind my son's back and have him push back. It works best when doing his P.T. exercises.
