Hello, The vacuum breaker goes onto your wall-outlet. The hose then connects to the vacuum breaker.

I am assuming that you have a vacuum break between the wall water outlet and the hose itself (required by code where I live). My guess is the vacuum break has worn out and needs to be replaced. To test this, just unscrew the hose and then unscrew the vacuum breaker, and then screw the hose directly to the water outlet....assuming that you have the rubber washer in place, the leak should be gone. So all you need to replaced would be the vacuum breaker. 5 years is about right as I have just submitted my request to Hansgrohe to get ours replaced for the same reason (Hansgrohe has a limited lifetime warranty on their vacuum breakers).

Does this come with any documentation to prove it meets asme 112.18 to prevent back-flows?

This is not a check valve or back flow preventer. It is simply a vacuum breaker which allows air in so that you can drain the hose if you hold the hand-held shower down so that it can drain.

What thread size for the inlet side and outlet side, and is it NTP?

It’s 1/2” NPT, female inlet, male outlet the same as your shower head/hose.
