Q: What prevents the head from turning when doing an angular torque rotation?

Nothing. Unlike other angle gauges this one doesn't need a fixed reference. Not sure how it works ( black magic I guess) but it works quite well.

Q: Does it matter if the unit is at the ratchet head or at the end of an extension?

Sir: I've used this tool to no end, and with or without a ratchet end, at the end of an extension, or maybe even on a speed wrench, the tool works great! The analog-style has its "quirks", but that will also be a good choice for checking degrees on spark plugs or even setting up your valve train and a small block Chevy.....whatever you're working to achieve, this tool is a good purchase.
