Does this move around on the toilet while using or is it secure? Seems like its flat & will move.

It is not flat. There is a part that is about an inch under what you see in the picture. That is what holds it in the bowl of the toilet.

How big is the opening at the bottom? Is it just a slot or is it the entire bottom open?

The entire bottom is open, a CD could easily fall through into the toilet (this has happened to me a couple times where it will slide down from clip while spraying and into toilet). I'm not sure of exact measurements of the opening.
